To become a patient:
An Oklahoma MD or DO (who meets the definition of “board certified” under rules established by either the Oklahoma Board of Medical Licensure or the Oklahoma Board of Osteopathic Examiners) may recommend medical cannabis to patients with any medical condition at their discretion.

Adult Patients:
Step 1:
Print and complete the Patient Information portion of the Physician Recommendation Form.
Step 2:
Schedule an appointment with your Board Certified MD or DO. During your visit present the Physician Recommendation Form for your doctor to complete and sign.
This is NOT a prescription, your doctor’s signature is simply certifying you have a medical condition and may receive therapeutic benefits from the use of medical cannabis. This form must be submitted to OMMA within 30 days of the physician’s signature to be valid.
*If your doctor refuses to certify this document- ask for copies of your medical history proving your diagnosis and schedule an appointment with one of the doctors listed on our Doctor Locator.
Step 3 (optional):
If you are homebound you may choose to designate a caregiver. Complete the Adult Patient Caregiver Designation Form. You may withdraw your designated caregiver at any time using the Patient Withdrawl of Caregiver Form.
Step 4:
Register with OMMA. Access the patient portal and create an account. Please note whichever email address designated to log in will be used by OMMA to send notices once the account is created.
The following information will be needed:
- Full name
- Residence and mailing address
- Date of birth
- A telephone number and email address
- Information about your physician
- Your signature dated within 30 days of when you submit your application
- Valid, unexpired documentation will also be submitted:
- Proof of Oklahoma residency
- Proof of identity
- A clear, color, full face digital photograph
- Adult Patient Physician Recommendation Form dated within 30 days of your application submission
- If applicable, your Medicaid (SoonerCare) or Medicare insurance card or enrollment documentation
- If applicable you will also submit the completed Adult Patient Caregiver Designation Form
- Proof of Oklahoma residency
- Proof of identity (digital copy of one of the following):
- A clear, color, full face digital photograph
The application fee is $100 (Medicaid patients will pay a reduced fee of $20) and will be paid using a Visa or MasterCard credit or debit card. This fee is non-refundable.
Step 7:
Receive an approval letter including your Patient Identification Card within 14 days of submitting the application.

Minor patients must also designate up to two caregivers to purchase, transport, or cultivate medical cannabis for them. Minor caregivers may be parents or legal guardians of the minor patient. Print and complete the Caregiver Designation Form.
Step 6:
Register the minor patient. Access the patient portal and create an account for the minor patient. Please note whichever email address designated to log in will be used by OMMA to send notices once the account is created.
The following information about the minor patient will be needed:
- Full name
- Residence and mailing address
- Date of birth
- A telephone number and email address (if applicable)
- Information about the two recommending physicians
The following information about the parents or legal guardian will be needed:
- Full name
- Residence and mailing address
- Date of birth
- A telephone number and email address
Unexpired documentation will also be submitted to OMMA:
- For the Minor Patient-
- Proof of identity
- A clear, color, full face digital photograph
- First Physician Recommendation Form
- Second Physician Recommendation Form dated within 30 days after the First Physician Recommendation Form and dated within 30 days before the application submission date
- If applicable, the minor’s Medicaid (SoonerCare) or Medicare insurance card or enrollment documentation
- For the Designated Caregiver:
- Proof of Oklahoma residency
- Proof of identity (digital copy of one of the following):
- A clear, color, full face digital photograph
- If applicable, documentation establishing legal guardianship

Yes I have Eplisy.I have broken my neck twice also was in a coma for all from seizures. I have them bad.i can’t deal with my pain from so many injuries to my neck and shoulders.

Will you be able to still take pain meds with this
This is a conversation you need to have with your doctor, but there is nothing in the law that restricts this.
The big hospital groups are already telling patients that pain meds will not be prescribed if the person is using medical marijuana
That’s unfortunate. We’ve seen the same thing in Arkansas.
They do the same thing in Montana
According to my pain doctor who is for marijuana, the health department will block doctors from being able to prescribe any controlled substances while your marijuana card is valid. That includes cough syrups, pain meds, testosterone, nerve medications, etc.. It’s up to the doctor that writes your recommendation to go over that with you. You will truly be on your own for 2 years.
This is what my dr is worried about. If he signs the recommendation form, will he be federally prohibited from continuing to prescribe pain meds. We need to be able to find out if mmj works before they’re required to cut us off our pain meds.
my son is 28, cant talk. he is mentaly of a 18 month old baby,. iam his mother and legal gaurdian by court. so I fill out the ” Adult Patient Caregiver” for my son since he cant and that will be ok ?

If I go to a therapist and talk to them and they determine I do have certain things on this list, will that count or you have to take that info to the doctor

You may use your driver’s license for both
Your document is fine, but it doesn’t mention that currently, all recommending physicians must be Nationally Board Certified. FYI.

On the Adult Step 4, there is a deadline to register with OMMA, which is 10 days from now. I do not believe appointments with both my PCP doctor and my Neurologist doctor can happen by that deadline let alone requesting medical records if needed. Options?
That’s not a deadline, tomorrow is when the portal for you to submit your documents to the state. As long as your documents are submitted within 20 days of your physician signing them you will be good.

You should be able to take your Kansas records to your Oklahoma Recommending Physician. Be sure to call the clinic before your appointment to verify.
Having problems trying to find a position that will sign off on the application so far of the positions that we have talk to or afraid of using losing their license to practice in there DEA license because of federal law on medical marijuana it’s very conflicting With federal law in a position holding a DEA license. Any suggestions would be appreciated
